29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): On, or about, March 14, 2019, the employer failed to implement and maintain a program that included frequent and regular inspections of the job site at the New South High School Construction Project located at 5000 Shankland Road, in Willoughby, Ohio, 44094 when employees were exposed to 15 feet falls while performing cleaning activities and were working near a second floor, double door wall opening, and were not protected from falls.
