29 CFR 1910.22(c): The employer did not provide, and ensure each employee used, a safe means of access and egress to and from walking-working surfaces. On or about June 6, 2019 and at times prior thereto, the employer did not provide a safe means of access and egress to employees performing work in the following areas: a. Climbing in/out of the elevated blenders to break blocks of fertilizer, a fixed ladder or work platform was not available, employees used totes and an 8foot ladder that did not safely reach to the top approximately 10 feet and was not at least 3 feet above;. b. Employees climbed from one old blender to another after accessing them through the single fixed ladder at the first blender; and c. Accessing and performing work on chemical trucks with out proper fall prevention.
