29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. a) On May 29, 2019 an employee was in the process of attaching grommets to bank bags utilizing an ELA Grommet machine. As the employee placed a bag onto the machine to attach the grommet, stepped on the foot pedal to engage the machine, the employee noted that the grommet did not attach properly, while trying to adjust the grommet, the employee engaged the foot pedal, and the ring guard came down catching the employees finger between the guard and the bottom section of the attachment area.
