29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Building 12 - On or about June 1, 2019, employees were exposed to a fall hazard when working from the third floor decking near an unguarded stairwell that was approximately 19'-8" above the ground, in that, employees were not being protected from falling by a guardrail system, safety net systems, or personal fall arrest systems. b) Building 12 - On or about June 1, 2019, the employer allowed employees to work from the roof truss beams that were approximately 9'-1" above the third floor decking without being protected from falling by a guardrail system, safety net systems, or personal fall arrest systems. c) Building 12 - On or about June 1, 2019, the employer allowed employees to remove the lanyard from the anchor strap and walk on the roof truss beams that were approximately 19'-8" above the second floor decking without being protected from falling by a guardrail system, safety net systems, or personal fall arrest systems. d) Building 12 - On or about June 1, 2019, employees were exposed to a 19'-8" and a 9'-1" fall hazard, in that, the anchor points used while working from the roof truss beams were inadequate. The truss section had not been restrained, braced and sheathed in accordance with the manufacturer's instructions to provide a suitable structure to establish an anchor point. CMET Enterprises, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1273716, citation number 1, item number 1 and was affirmed as a final order on 2/21/18, with respect to a workplace located at 215 Anastasia Boulevard, Saint Augustine, FL 32080.
