29 CFR 1926.1404(e)(2): When assembly/disassembly crew members were out of operator view, the operator did not ensure that any part of the equipment (or load) was moved until the operator was informed that the crew member was in a safe position: a) On or about May 22, 2019 located at SHP2 Frac Project, a crew member was exposed to a struck-by hazard when out of view of the crane operator as the counterweight was being set.
