On or about and times prior to: 5/31/2019 Location: Production floor, North side a) The employer modified two mobile scaffolds; attaching a water plumbing system and used in such a way that it was not intended by the manufacturer. Employees were exposed to struck-by and caught-between hazards from overturning.
