29 CFR 1910.146(d)(4)(iv): Under the permit-required confined space program required by 29 CFR 1910.146(c)(4), the employer did not provide personal protective equipment in so far as feasible engineering and work practice controls did not adequately protect employees: a) On or about May 24, 2019 at Bay 2 of the 704 area, employees did not use effective personal protective equipment when cleaning out tank 1155 that contained aniline waste.
