29 CFR 1926.453(a)(2): Aerial lifts were "field modified" for uses other than those intended by the manufacturer, the modification had not been certified in writing by the manufacturer or by any other equivalent entity. Modification did not conform with all applicable provisions of ANSI A92.2-1969: On or about May 29, 2019, the employer failed to obtained the manufacture's approval before modifying the mid-rail of the basket of the Snorkel articulating boom lift: model #TB-66JDZ, serial #SP06177. The mid-rail was taped to the top-rail which allowed employees to crawl in and out of the lift's basket. The component was not replaced with a gate or similar device, and although employees were don in harnesses and lanyards; they were not tied off and were thus exposed to a fall of approximately 15(+) ft.
