29 CFR 1904.29(b)(2): The employer did not complete or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness. On or about March 25, 2019, an employee was hospitalized after being found on the ground from an apparent loss of consciousness and fall to a lower level. (a) The employer did not complete block 9 of the OSHA 301 form, identifying that the employee was hospitalized. (b) Loss of consciousness is an OSHA recordable injury. The employer did not record this injury on their OSHA 300 log and placed an N/A in block 10 of the OSHA 301. (c) The employer did not complete block 12 of the OSHA 301 form, identifying the time the employee began work. (d) The employer did not properly fill out block 16 in that, the employer placed an "n/a" in the block. (e) The employer did not properly fill out block 17 in that, the employer placed an "n/a" in the block. NOTE: Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos, which you believe will be helpful. The abatement certification sheet is enclosed with the citation(s).
