29 CFR 1910.134(k)(6): The basic advisory information on respirators, as presented in Appendix D of this section, was not provided by the employer in any written or oral format, to employees who wear respirators when such use is not required by this section or by the employer: On or about May 22, 2019, and at times prior thereto, at a workplace located at 1500 Heritage Parkway, Mansfield, Texas; the employer did not provide the basic advisory information on respirators, as presented in Appendix D of this section, in any written or oral format, to employees who wear respirators when such use was not required by this section or by the employer. Employees were potentially exposed to respiratory hazards when performing tasks such as, but not limited to filling tires with polyurethane compounds.
