Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. Employees are allowed to operate the Chicago Model 135 Press Brake with an unprotected foot pedal. Employees are exposed to unexpected machine operation and amputations. The employer is failing to protect employees from amputation hazards. On or about May 21, 2019, an employee operating the Chicago Dreis & Krump Model 135 Press Brake received an amputation when unintentionally stepping on the foot pedal for initiating Press Brake operation. Among other methods, one feasible and acceptable abatement method is to: a) Guard the foot pedal to prevent unintended operation in accordance with American National Standard ANSI B11.3-2012. b) Provide a two-hand method of actuation available for operator use.
