29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group: On or about May 16, 2019 and at times prior thereto: a. In the pallet inspection area of SMETCO #1 pallet sorting machine where employees were exposed to struck-by and caught-by hazards while working on and around the pallet sorting machine that was de-energize without being locked out by all group members doing maintenance or servicing work on the machine. b. In the pallet inspection area of SMETCO #2 pallet sorting machine where employees were exposed to struck-by and caught-by hazards while working on and around the pallet sorting machine that was de-energize without being locked out by all group members doing maintenance or servicing work on the machine.
