29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) Roof of B1 addition - On or about May 8, 2019 and at times prior thereto, employees were working around an unprotected floor opening measuring 33"x33" approximately 20' above the lower level. On May 8, 2019 an employee was hospitalized with multiple fractures following a fall through the floor hole.
