Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards while using compressed air to clear a plug from a concrete hose. a) On or about May 3, 2019, at the worksite located at 2768 Front Street, Toledo, OH: An employee was standing on a two inch rubber concrete hose, attempting to loosen an obstruction with a hammer as another employee was manually applying compressed air to the concrete hose. The plug released, causing the hose to whip around striking the employee in the leg resulting in a serious injury. One feasible and acceptable method to correct this hazard is to utilize ANSI B30.27 - 2014, Material Placement Systems, section 27-3.1.7 Cleaning Pipeline.
