29 CFR 1910.22(a)(1): (1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. On or about May 7th, 2019 employees were exposed to struck by hazards, caught between hazards, falls from the same level, and toxic substance inhalation exposure to airborne mercury vapor while working where facility maintenance supplies and equipment was stacked and stored in a disorderly condition whereas in: Instance 1: the men's locker room; boxes of maintenance supplies were stacked over six feet from the floor, unsteady, and large, heavy boxes were stored on smaller boxes above and adjacent to the lockers Instance 2: the server room where card board boxes were stacked and staged throughout the room completely blocking the walking surface, blocking access to the thermogenic network server, and blocking clear access to the portable 14 fan used to continuously reduce the temperature of the server which was energized from a medium duty approximately 100 extension cord that was being used as permanent wiring and which was coiled and spread out around the room under the stored card board boxes Instance 3: the men's locker room where over 12 used, mercury laden fluorescent light bulbs were unsecured, leaning against a wall, in the passageway to the server room
