29 CFR 1915.73(b): When employees were working in the vicinity of flush manholes and other small openings of comparable size in the deck and other working surfaces, such openings were not suitably covered or guarded to a height of not less than 30 inches: A. On or about May 7, 2019, USS John Warner, Graving Dock #2, Sonar Dome: Employees performing pressure washing activities on the fourth level work platform were not protected from falls through unguarded access and lightening holes. A painter, engaged in pressure washing operations, with limited visibility, stepped through an unguarded access hole and fell more than 20 feet to the lower level, sustaining serious injury.
