29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Fluidized Sand Bed Cleaning Area - On or about April 22, 2019, the employer did not develop and document specific lockout procedures for the Seghers Better Technology USA, Fluidized Sand Bed Cleaning System, Model #M4657D. The Fluidized Sand Bed Cleaning System had two or more sources of energy (electric and gravity). The system could not be completely deenergized and deactivated by a single lockout device. The lack of a procedure that clearly and specifically outlined the techniques to be utilized to lockout the machine exposed employees to uncontrolled sources of energy.
