29 CFR 1926.20(b)(2): The employer did not ensure that frequent and regular inspections of the job sites, materials, and equipment were made by competent persons: a) Job site, 1220 Western Avenue, Pittsburgh, PA: On or about April 22, 2019, during construction activities, the employer did not ensure that the job site work conditions were properly overseen in order to identify and correct safety concerns. Deficiencies noted during the inspection included, but were not limited to, forklift loads were not lowered to the ground when the forklift was not in operation and appropriate fall protection practices were not in use where fall hazards of up to at least 40 feet existed.
