29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Planer Mill Area (In-Feed Area): On or about April 16, 2019 and at times prior thereto; the employer exposed employees to caught-in and struck-by hazards in that the employer failed to ensure employees utilized the hazardous energy control procedure when employees were engaged in servicing and maintenance activities, including but not limited to clearing lumber jams in and around the pressure and pineapple rolls at the infeed conveyor system.
