OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or were likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards: a) On or about April 6, 2019; An operator of a Toyota sit-down type LP-gas model 7FGU45, SN # 61243 was observed not wearing the attached seatbelt, thus exposing the operator to crushing injuries associated with tip over hazards, on or about April 18, 2019.
