29 CFR 1910.95(g)(6): At least annually after obtaining the baseline audiogram, the employer did not obtain a new audiogram for each employee exposed at or above an 8-hour time-weighted average of 85 decibels: a) On or about April 3, 2019- Stacking and Drying Area, Goal Post Groundman, the employer did not obtain a valid audiogram within a year of the baseline audiogram for employees exposed at or above the action level of 85 dbA as an 8 hour time-weighted average. The employee was hired in April 2015, received a baseline audiogram, but had not received an audiogram in 2016 or 2018. b) On or about April 3, 2019- Sawmill, Green Stacker Operator, the employer did not obtain a valid audiogram within a year of the baseline audiogram for employees exposed at or above the action level of 85 dbA as an 8 hour time-weighted average. An employee working as a Stacker Operator was exposed to 188.1% of the permissible daily noise exposure of an 8 hour time-weighted average of 90 dBA. Sampling was performed for 7 hours and 24 minutes on 4/4/19, with the unsampled portion of time considered to represent zero exposure. The employee was hired in October 2016, received a baseline audiogram, but had not received an audiogram in 2017 or 2018. c) On or about April 3, 2019- Sawmill, Relief Operator, the employer did not obtain a valid audiogram within a year of the baseline audiogram for employees exposed at or above the action level of 85 dbA as an 8 hour time-weighted average. The employee was hired in August 2017, received a baseline audiogram, but had not received an audiogram in 2018. d) On or about April 3, 2019- Sawmill, Supervisor, the employer did not obtain a valid audiogram within a year of the baseline audiogram for employees exposed at or above the action level of 85 dbA as an 8 hour time-weighted average. The employee was hired in July 2015, received a baseline audiogram, but had not received an audiogram in 2017 or 2018.
