29 CFR 1910.28(b)(3)(iv): Each employee was not protected from falling into a ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, and where a self-closing gate or an offset was not used. On or about April 2, 2019, on the platform of the filter press area (Cake #3), employees operating the filter press were exposed to a fall hazard exceeding four feet where the ladderway was not equipped with a gate or offset.
