29 CFR 1910.147(a)(2)(ii)(A): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee (s) was required to (A) Remove or bypass a guard or other safety device; (B) were required to place any part of his or her body into area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone existed during a machine operating cycle: (a) CFM LINE 2 - Employees changed out and adjusted the tension on the slitter blades on the slitter portion of the machine while the machine was running in normal production mode, which exposed the employees to the in-running nip point created at the compaction rollers located in this area of the machine, on or about March 26, 2019. (b) CFM LINE 3 - Employees changed out and adjusted the tension on the slitter blades on the slitter portion of the machine while the machine was running in normal production mode, which exposed the employees to the in-running nip point created at the compaction rollers located in this area of the machine, on or about March 26, 2019. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
