At the residential home located at 1110 Harbor Haven, Southlake, Texas, the employer did not initiate a program to perform frequent and regular inspections of the MAYCO and Western plaster pumps, and their components including but not limited to hoses, mixers, couplings, and nozzles, exposing employees to struck-by hazards.
