29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation: a. On or about March 12, 2019, all employees required to service and maintain the Rotary Molder Machine, serial # MM001-048-2017-9914, and other machinery were not provided training as authorized employees, thereby exposing employees to amputation hazards.
