29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of the incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on OSHAs public web site at www.osha.gov: a) U.S. Postal Service - The employer did not report an in-patient hospitalization that resulted from a work-related incident on or about February 26, 2019, to OSHA within twenty-four (24) hours. The injury was reported to OSHA on March 6, 2019. U.S. Postal Service, was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1904.39(a)(2), which was contained in OSHA Inspection Number 1286249, Citation 1, Item 1, and was affirmed as a final order on May 30, 2018, with respect to a workplace located at 1250 Smile Way, York, PA, 17404. U.S. Postal Service, was previously cited for a violation of this Occupational Safety and Health Standard, 29 CFR 1904.39(a)(2), which was contained in OSHA Inspection Number 1288391, Citation 1, Item 1, and was affirmed as a final order on June 8, 2018, with respect to a workplace located at 6450 Kings Highway, Zionsville, PA, 18092.
