29 CFR 1926.602(d): Powered industrial truck operator training. Note: The requirements applicable to construction work under this paragraph are identical to those set forth at '1910.178(l) of this chapter. NOTE: 29 CFR 1910.178(I)(6) Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. On or about March 19, 2019, employee was not certified in the operation of the rough terrain power industrial truck exposing employee to caught in and crushed hazard.
