Text For Citation: 01 Item/Group: 001 Hazard:
OSH Act of 1970 Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck-by heavy equipment: (a) Department 126, Poletype Division, Regulator Tank Shop, Leak Test and Repair - On or about March 15, 2019, the employer did not ensure adequate type, maintenance, or inspection of the roller chains that were used for the chain drive system on the Regulator Tank Shop, Leak Test and Repair equipment to prevent employees from being struck-by or crushed by the equipment. (b) Department 126, Poletype Division, Regulator Tank Shop, Leak Test and Repair - On or about March 15, 2019, the employer did not ensure adequate type, or inspection of the shackle that was attached to the safety chain used for locking out the gravitational forces applied to the Regulator Tank Shop, Leak Test and Repair equipment, exposing employees to struck-by hazards.
