Text For Citation: 01 Item/Group: 001 Hazard:
OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: a) 3 Terminal Road, New Brunswick, NJ - Tablet Press/Encapsulation Department - Dust Collection System containing Class II combustible dietary supplement dust: Employees are exposed to the hazard of fire and/or explosion resulting from the ignition of Class II, explosive dusts in and around process equipment such as but not limited to the tablet press machine(s) and dust collection system(s) because the equipment was not adequately inspected, cleaned, serviced and maintained: 1) A dust hazard assessment and evaluation were not conducted to determine whether dust explosion hazards or flash-fire hazards exist in areas where accumulations exist external to equipment. 2) Equipment such as but not limited to tablet press machines were not maintained and operated in a manner that minimized the escape of dust. 3) The dust collection system(s) was not provided with a deflagration suppression system. 4) Isolation devices were not used to prevent deflagration propagation between connected equipment such as but not limited to tablet press machines and/or work areas. 5) Ventilation duct systems were not all grounded and bonded. 6) All other conductive equipment components such as but not limited to the tablet press machine(s) and the dust collection system(s) were not grounded and bonded. 7) The dust collection system(s) was not provided with a spark detection system. 8) A written emergency response plan was not developed for work-related emergencies including but not limited to fire and explosion. 9) Employees were not provided training on operating, maintaining, and working with combustible particulate solids. Condition observed on or about 3/15/2019.
