29 CFR 1910.28(b)(3)(iv): (iv) Each employee is protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used; (a) Employees were exposed to fall hazards of approximately 6 feet while working on the ladderway platform in front of the Koger Model 16-2-33 dust cyclone entrance hatch that was not provided a self-closing gate or an offset as required. (b) Employees were exposed to fall hazards of approximately 12 feet while working on the Rodrigue Metal Ltee abort gate ladderway platform that was not provided a self-closing gate or an offset as required. (c) Employees were exposed to fall hazards of approximately 6 feet while working on the trailer ladderway platform that was not provided a self-closing gate or offset as required.
