29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: On or about February 27, 2019, an employee suffered an injury which required admission to a medical facility and subsequent surgery and the employer did not report the incident to OSHA within 24 hours. HELP AT HOME, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF AN EQUIVALENT OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1904.39(b)(7), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1177669, CITATION NUMBER 1, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON DECEMBER 13, 2016, WITH RESPECT TO A WORKPLACE LOCATED AT 647-1/2 W LOSEY STREET, GALESBURG, ILLINOIS 61401.
