29 CFR 1910.27(b)(2)(ii): The employer did not ensure rope descent systems were used in accordance with the manufacturer's instructions, warnings, and design limitations or under the direction of a qualified person. Location: Job site The company did not follow rope descent system manufacturers instructions. Instance A: The outrigger gantry head was not secured to the gantry frame or beam. Instance B: A parapet wall was used to 'anchor' a rope descent system without the wall having been structurally analyzed by a registered professional engineer. Instance C: An MIO parapet clamp tie-back was not directly behind the outrigger that it 'served'. Instance D: An MIO parapet clamp was tightened using a pipe instead of hand tightened as required by the manufacturer. Instance E: Rope grabs and rope were not being cleaned in accordance with the manufacturers recommendations.
