29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator was involved in an accident or near-miss incident: The employer is failing to protect employees from forklift accidents by not providing adequate follow-up refresher training following a forklift accident. This was most recently observed at 71045 NE 50, Pawnee City, Nebraska on February 28, 2019 when an employee who had been involved in a forklift accident had not received refresher training prior to operating a forklift again.
