29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: The employer is failing to protect employees from engulfment hazards associated with trenching and excavations. This was most recently documented on or about February 14, 2019 at a jobsite located at or near: Ivyleigh Court, St. Charles, Missouri. Employees were installing sanitary sewer pipe in a trench measuring a depth ranging from 13.0 -14.0 feet, with near vertical walls. The exposed employee was installing an 8 foot section of 8 inch PVC pipe more than 35 feet away from an earthen ramp.
