OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crush and amputation hazards: a) 1315 Hickman Avenue, Elba AL; On or about February 7, 2019 and at times prior thereto the employer exposed employees to crush and amputation hazards in that they were required to operate a Cincinnati 250 Ton Mechanical Press Brake with only one foot pedal for two operators.
