29 CFR 1904.39(b)(7): The employer did not report to OSHA an in-patient hospitalization, amputation or loss of an eye within 24-hours after the employer was notified of the in-patient hospitalization, amputation or loss of an eye: a) 1411 Lancaster Ave - An employee injury resulted in amputation and in patient hospitalization on December 12, 2018. The employer did not notify OSHA of the in patient hospitalization within 24 hours of learning of the employees hospitalization. The company notified OSHA on January 28, 2019. No abatement certification or documentation required.
