29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about 2/11/2019, at the truck shop located at 110 Paradise Road, Athens, GA and the farmhouses located at 375 or 433 Sandy Cross Road, Lexington, GA: Employees performing servicing and maintenance activities on powered industrial trucks, field trucks and trailers, were exposed to struck-by and crushing hazards, in that; no procedures were developed to control the gravitational energy sources such as but not limited to the mast of the powered industrial truck when elevated.
