29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: a) Paulsboro Refining Company, located at 800 Billingsport Road, Paulsboro NJ, FCC Complex: The employer failed to develop and implement a written procedure for operators to utilize when clearing pluggages from bleeder valves, on or about 2/6/2019. b) Paulsboro Refining Company, located at 800 Billingsport Road, Paulsboro NJ, FCC Complex: The employer did not develop a safe work practice for operators which considered the configuration of the piping and the effectiveness of the personal protective equipment which would be used, on or about 2/6/2019. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
