29 CFR 1910.146(k)(3)(i): Each authorized entrant did not use a chest or full body harness, with a retrieval line attached at the center of the entrant's back near shoulder level, or above the entrant's head: a) At the Atomizer, on or about January 30, 2019 - An Authorized entrant did not have a retrieval line attached to the employee. On January 30, 2019, an employee entered the Atomizer to check for a leak in the hydraulic system and did not have a retrieval line attached to the harness.
