29 CFR 1910.23(b)(9): (9) Ladders were not inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury: a) Building Interior-Warehouse: An employee was using an extension ladder that was not inspected before each use and more frequently as necessary to identify any visible defects that could cause employee injury. On 1/16/2019, an employee sustained injuries to both of her feet, when she fell from a ladder that was not inspected prior to being used. The ladder was found to have defective anti-slip safety shoes/feet.
