Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about January 16, 2019 an employee was operating an ACMA ravioli forming machine and reached his hand through an opening that was approximately 2 ½ inches from the conveyor to the guard to remove a dry piece of pasta on the stamp. The employee came into contact with the cutting bar which resulted in an amputation at the time of the accident due to a lack of guarding on the machine. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
