29 CFR 1910.147(e)(3): Each lockout or tagout device was not removed from each energy isolating device by the employee who had applied the device: The employer is failing to ensure that employees are not exposed to caught-in or between hazards. This was most recently documented on January 24th 2019 at 8648 S 117th St. La Vista, Nebraska. The employer failed to ensure that the sharing of keys was not occurring allowing for employees to remove isolating devices that they had not applied.
