29 CFR 1910.212(a)(3)(iii): Special handtools for placing and removing material(s) was not provided to permit easy handling of material without the operator placing a hand in the danger zone: On or about 1/11/2019, in the shipping department, an adequate tool to hold the material to be cut was not provided where a worker was cutting a piece of foam using a Moak band saw (S/N 10060) . On the date above, the worker's tip of the left thumb was cut off when it made contact with the band saw.
