29 CFR 1910.119(f)(3): Operating Procedures were not reviewed as often as necessary and certified annually to assure that they reflect current operating practice and accurate. This violation was most recently observed on or about January 8, 2019, in the Refrigeration Process Unit ER #2, when the employer failed to review as often as necessary and update the following standard operating procedures (SOPs): c) The CP4 Standard Operating Procedure Emergency Shutdown procedure called out CP2-1, CP2-2 and CP2-6. It should be CP4-1, CP4-2 and CP4-6.
