29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a. On or about 11/15/2018 at Hunters green community in Tampa, Florida, an employee suffered a work-related amputation and was hospitalized. The employer was aware of the in-patient hospitalization and amputation on 11/15/2018. The employer notified OSHA on 12/07/2018 at 3:38 p.m. of the in-patient hospitalization and amputation.
