29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level: A) Henry Gurtzweiler, Inc., conducting work at 1001 Park Avenue, Fostoria, OH 44830: On or about January 15, 2019, the employer did not ensure that the 24-foot fiberglass extension ladder was secured in place to prevent accidental displacement when an employee was transitioning from the ledge to the roof of the building. Employees were exposed to fall hazards of up to 15 feet.
