29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a) 10660 Page Ave., Fairfax, VA 22030: On January 14, 2019, at approximately 5:22 p.m., an employee suffered injuries from a slip/fall from the same level. The incident resulted in an in-patient hospitalization on the same day. The employer did not report the in-patient hospitalization to the Occupational Safety & Health Administration within the 24-hour reporting period. United States Postal Service was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1910.39(a)(2) which was contained in OSHA inspection number 1097986, citation number 1, item number 1, issued on October 16, 2015, with respect to a workplace located at 2905 Mars Street, Virginia Beach, VA 23452 and was affirmed as final order on November 16, 2015.
