Text For Citation: 01 Item/Group: 001 Hazard:
29 CFR 1910.212(a)(3)(ii): Point of operation of machinery were not guarded or guards were not designed and constructed to prevent employee(s) from having any part of their body in the danger zone during operating cycles: (a) At the workshop - On or about and at times prior to January 11, 2019, an employee actuated the foot pedal causing the brake press to cycle while attempting to flip to backstop of the dye causing crushing injuries on the right hand. The employer did not ensure that the point of operation regarding the Cincinnati Series 9 (serial # 38343) brake press was effectively guarded. This condition exposed employees to crushing and amputation hazards.
