29 CFR 1910.28(b)(1)(i): (1) Unprotected sides and edges. (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: a. On or about January 7, 2019, in the injection area, the employer failed to ensure that employees who perform activities on top of injection molding machines such as clearing/cleaning out solidified nylon plastic material and Asaclean from the feed throat and barrel on the I33 Haitian injection molding machine (model: MA6000; S/N: 201107060034995) were protected from falls 4 feet or more above a lower level. On or about January 7, 2019, an employee performing the aforementioned task was exposed to a fall hazard of approximately 6 feet, 1/16 inch and was forced away and onto the ground when liquefied nylon, gaseous, and/or other material was released through the feed throat due to pressure buildup.
