29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system. On or about January 4, 2019 an employee was performing cleaning activities on top of the vessel known as the let tank. The employee fell approximately 9 feet 7 inches to the ground resulting in hospitalization. The employer did not provide protection for the employee being exposed to falls from unprotected edges. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
